• W2294939098 abstract "Dementia is considerably underdiagnosed in primary care. Routine screening for dementia in general practitioner (GP) practices increases the formal diagnosis rate up to 70%. However, screening is not recommended by current guidelines because there is a lack of evidence regarding the harms and benefits of screening. Main objective of the present analysis is to determine the effect of receiving a formal dementia diagnosis after screening on relevant patient variables. DelpHi-MV (Dementia: life- and person-centered help in Mecklenburg-Western Pomerania) is an on-going GP-based randomized controlled intervention trial to implement and evaluate an innovative concept of collaborative care management in Germany. Present analyzes are based on data of 231 positively screened patients (age 70+, living at home) who had not been formally diagnosed with dementia before the screening (January 2015; preliminary data). The medical diagnoses were retrieved from the patient's medical records. Depression (Geriatric Depression Scale; GDS), quality of life in Alzheimer's Disease (Quol-AD), knowledge about the disease dementia, and antidementia drug treatment have been assessed at baseline. Prior to the baseline assessment, 113 out of 231 patients (49%) were formally diagnosed with dementia after they have been screened positive. Univariate comparisons showed no statistically significant differences between formally and not formally diagnosed patients regarding the potential harms of screening: depression (GDS≥6: 13% vs. 15%; p=0.710); quality of life (M=2.79 (SD=0.32) vs. M=2.75 (SD=0.45); p=0.389). Concerning potential benefits there was a statistically significant higher antidementia drug treatment rate among patients who had been formally diagnosed (20% vs. 11%; p=0.038 (one-sided)). The knowledge about dementia did not differ between the two groups (78% vs. 75% stated that they know the disease dementia; p=0.756). Multivariate analyzes confirmed the findings for depression, quality of life and knowledge of dementia but not for antidementia drug treatment. Present results suggest that receiving a formal diagnosis of dementia after a positive screening outcome does not have negative effects on the patients’ mood or their quality of life. However, there might be a benefit of being recognized as cognitive impaired and subsequently formally diagnosed with dementia regarding the antidementia drug treatment." @default.
