- W2306908968 abstract "1975 Objectives Pyogenic spondylodiscitis is often resistant to antibiotic therapy, which requires surgical intervention. However, targeting surgical fields is often difficult with anatomical imaging due to multiple chronic remodeling processes. The purpose of this study was to investigate clinical utility of FDG-PET/CT to establish surgical targets & indication, and to evaluate therapeutic efficacy of minimally invasive endoscopic surgery. Methods Six patients with pyogenic spinal infection underwent whole-body FDG PET/CT before surgery, and 1 & 6 months after the final surgery. Surgical fields were determined using established clinical modalities with a reference to the FDG PET/CT. Seven ROIs were set in the subdivision of spine and its surrounding structures at each disc level from L2 to S1 (35 ROIs per patient). The findings of FDG-PET/CT and its uptake (SUVmax) were correlated with surgical findings, histological findings, bacterial cultures, and other clinical parameters. Results Surgery was performed in 44/210 ROIs. Among 44 ROIs, 32 showed abnormal FDG uptake before surgery. Surgical findings and histology revealed active infections in 24/44 ROIs. Bacterial culture was positive in all patients. Histological correlation showed active infection with SUVmax of 6.4 ± 2.1(3.0-12.7). ROC analysis revealed the optimal cut-off at SUVmax of 4.2 (sensitivity 92% specificity 88.9% AUC 0.953).
