A strong correlationi'as found, suggesting that the mea- surement of tear lithium concentrations may be a viable alternative to plasma lithium determinations in some pa- tients. The use of blood levels as a means of monitoring drug levels is standard clinical practice for patients undergoing lithium carbonate treatment. Because the therapeutic-toxic index is rather narrow, this monitor- ing sometimes needs to be done frequently, especially in the initial phases of treatment. In some patients, however, venipunctures, especially repeated veni- punctures, may not be desirable. The disadvantages of venipunctures include the possibility of a hematoma at the venipuncture site and/or difficulty in finding a vein in some patients. In addition, blood drawing is experi- enced by some patients as physically uncomfortable, and the necessary frequency of this test procedure may lead to their diminished compliance and reluc- tance to accept lithium prophylaxis.
