Important distinction is made between local control stimulation and remote control stimulation, as well as the concept of open loop stimulation vs. closed loop stimulation. The authors have appropriately included work of Irving Cooper, who became notable in Stereotactic and Functional Neurosurgery history for accidentally damaging the anterior choroidal artery during a pedunculotomy procedure, which lead to the realization that specific lesioning of deep brain structures such as basal ganglia and thalamus could alleviate tremor and other movement disorders. As the authors point out, Cooper also had initially hypothesized that chronic cerebellar stimulation capitalize on widespread Purkinje cell inhibitory output to control excitatory network responsible for seizure, and later postulated that the effects were instead linked to activation of the reticular formation and inhibition of the thalamus, thus supporting the overall concept that modulation of specific neural network systems would be able to treat epilepsy. This is an important point in that stimulation of areas of brain that is not directly involved in the seizure generation could abort seizures. With this in mind, the authors do a nice job of discussing neural networks such as the nigral control system, cortico-subthalamic-nigral circuit, and thalamocortical network as they relate to deep brain stimulation (DBS) and epilepsy." @default.
