- W2313377423 abstract "体外循環（cardio-pulmonary bypass：CPB）中、希釈による膠質浸透圧の低下や人工材料あるいは空気との接触などにより補体が活性化される。補体の活性化は生体防御機構により血管膜透過性を亢進させ、呼吸機能に重大な影響を与える。 そこで我々は、CPB中の因子がCPB前後での酸素加能に影響を与えるか検討した。対象は2013年3月から8月までの除外症例除く17例で、CPB導入前の動脈血液ガス分析とCPB離脱直後の動脈血液ガス分析結果を用いて、CPB前後での酸素分圧/吸入気酸素濃度（P/F比）の変化率（⊿P/F）と肺胞気動脈血酸素分圧較差（A-aDO2）の変化率（⊿A-aDO2）を算出した。検討内容は、⊿P/F・⊿A-aDO2とCPB中心筋保護注入後、開始60分後、離脱直前の3点における白血球平均値（WC-AV）、顆粒球平均値（GR-AV）、膠質浸透圧平均値（COP-AV）、液量平衡、浸透圧平均値（OSM-AV）、CPB時間、大動脈遮断時間における関連性について相関係数を用いて検討した。⊿P/Fと⊿A-aDO2に対し相関関係を示したのは、⊿P/F対GR-AV：R＝0.746（P＜0.05）、A-aDO2対GR-AV：R＝0.656（P＜0.05）と負の相関関係が得られた。⊿P/F対COP-AV：R＝0.469（P＜0.05）、A-aDO2対COP-AV：R＝0.606（P＜0.05）と正の相関関係が得られた。体外循環離脱直後での酸素加能の変化は、CPBによる侵襲が好中球活性とCPBによる希釈後のCOP低下の双方が肺血管外水分量を増大させ肺の透過性亢進による拡散障害が原因であると考えられた。" @default.
