- W2313409823 abstract "To demonstrate the usefulness and effectiveness in using the saphenous vein as a venous access site for the placement of peripherally inserted central catheters in young children. From November 2010 to October 2011, 18 children (11 male: 7 female) ranging from age 5 days to 4380 days (mean age of 360) at our institution underwent saphenous peripherally inserted central catheter placement. All catheters were placed in the ankle or thigh using ultrasound guidance and/or clinical landmarks. Immediate and short-term complications reviewed. Of 18 patients, 16 were less than 4 months of age with a mean of 40 days (2 patients were older being 4 and 12 years-old). Indications for the studies were urinary tract infection in 8, congenital heart disease in 6, and meningitis in 4. The technical aspects of the procedure included success in obtaining central venous access in 17 of 18 patients, with 1 failure in a saphenous vein measuring 1 mm. The mean catheter length was 28.7 cm, which included saphenous venous access at the thigh and ankle. All ankle venous access sites were in patients weighing less than 10 kg with a measn catheter length of 33.1 cm. Thigh venous access sites included older children and had a mean catheter length of 27.4 cm. A total of 284 catheter days was reviewed without reported complications. In 16 of 18, a 3 French catheter was placed, with remaining 2 patients receiving 4 French catheters. The catheter tips placed at T9 or T10 in 16 of 18. One patient had catheter tip at T8 and 1 patient had tip at T11. In children with accessibility to the saphenous vein either by ultrasound guidance and/or clinical landmarks, the saphenous vein provides a suitable alternative site for venous access in very young children and in a select group of older children that has not been previously reported." @default.
