- W2313771870 abstract "To develop a Pharmaceutical Care Certificate Program (PCCP) model for training community pharmacists in the provision of pharmaceutical care. Chain, independent, and HMO pharmacy practice environments. A two-year pilot program was used to create a nine-month training program that focuses on four areas: (1) practice re-engineering; (2) using the components of pharmaceutical care to correct drug-related problems; (3) drug therapy management of disease states; and (4) documenting the processes used and the care provided. The intent of the PCCP model is to enhance the ability of the participants to make the transition to pharmaceutical care by incorporating the components of the PCCP into their practice. Patient case exercises, reports on overcoming barriers, and practice application assignments on correcting drug-related problems. These materials are evaluated by the program director for content and approach, with particular emphasis on intervention and follow-up. Based on observations from site visits by the program director and his evaluation of participants' portfolios, PCCP pharmacists have become proficient at using the components of pharmaceutical care and the principles of drug therapy management to correct drug-related problems. They also have been able to make adjustments in workflow and physical layout to improve patient care. However, time to provide these services on a consistent basis remains a formidable barrier. Partnering with schools of pharmacy for PharmD clerkship training is an excellent way to advance practice toward pharmaceutical care. Schools of pharmacy need pharmacists who will model pharmaceutical care practice behaviors for students; pharmacists need students who can spend the time needed to assess patient problems and generate care plans, and do the developmental work for pharmaceutical care services." @default.
