- W2313785963 abstract "がん患者の精神症状（不安・抑うつ・せん妄など）は，患者の置かれている状況や身体的苦痛に大きく影響されている．患者にとって「悪い知らせ」を受けた直後の状況では，「頭の中が真っ白」であり意思疎通が困難となっている．この状況では，時間をかけた傾聴や助言も患者にとっては夢の中の他人事でしかないが，状況への正しい理解があれば，焦ることなく患者の気持ちに寄り添うことができる．また，「抑うつ」とがんによる身体的苦痛（疼痛・全身倦怠感など）が同時にある場合は，先ず身体的苦痛の緩和を図ることを優先すべきである．身体的苦痛の緩和のみで「抑うつ」が軽快・消失することが多いが，身体的苦痛の緩和なしに，抗うつ薬の処方や精神療法のみでは「抑うつ」は改善しない．さらに，「せん妄」においては，その精神症状の原因のほとんどが環境状況や身体症状に由来するので，これらを先ず治療・改善すべきであり，安易な抗不安薬・睡眠導入薬の処方は避けるべきである．以上のように，がん患者の精神症状に対する緩和ケアの秘訣と心得として重要なことは，「患者の置かれている状況や身体的苦痛を充分に理解し，先ずこれらを治療・改善すること」であるといえる．" @default.
