- W2314357645 abstract "To systematically evaluate the predictive value of ultrasound findings to ascertain if clinically significant retained placental tissue exists. A search of the MEDLINE database for all articles reporting ultrasonographic (both transabdominal as well as transvaginal) features in patients that were evaluated for retained products of conception. Studies were included if ultrasound findings as well as pathology specimens were reported for all patients; this approach was utilized so that positive and negative predictive values (PPV, NPV) could be reported. Fifty one articles were obtained from an initial search result of 217 citations; 14 of these met inclusion criteria. These were then compared according to type of cohort (spontaneous miscarriages, post termination of pregnancy, postpartum) and reported findings, with positive and negative predictive values ascertained. All studies reported on the appearance of the endometrial echogenicity, and 10/14 assessed the thickness of the endometrium (the only OBJECTIVES: indicator examined). Positive predictive values (PPV) were assessed from the reported results of each study, with a range of 29 to 100 % PPV depending on the findings reported, technique employed (including Doppler studies), and operator interpretation. Negative predictive value (NPV) ranges were found to vary from 15 to 100 %. There is a wide range of reported PPV and NPV regarding the use of ultrasound to evaluate for retained placental tissue, with only subjective findings (i.e. heterogeneity and/or echogenicity of the endometrium) demonstrating statistical significance. These results suggest that ultrasound findings alone have a limited role in predicting retained placental tissue, and should be utilized only as a part of an evaluation rather than as a diagnostic exam." @default.
