- W2314862598 abstract "今回,狭窄・膿瘍形成の為に手術を行った上行結腸憩室炎に多発性colonic muco-submucosal elongated polyp (CMSEP)を合併した症例を経験したので,文献的考察を加えて報告する. 症例は35歳男性.右腹部痛を主訴に当科を受診.大腸憩室炎を疑い保存的加療を行った.大腸内視鏡で盲腸の多発性ポリープと狭窄を,小腸造影で回腸側に縦走潰瘍を認め,クローン病による回盲部狭窄を疑い手術を行った.回盲部に約7cm大の膿瘍を認め,これを含めて回盲部切除を行った.切除標本では,瘻孔を認め,盲腸に約1.5～2.0cm大の舌様のポリープを6個認めた.病理組織では,憩室の多発と憩室炎による膿瘍形成と診断された.一方,ポリープは表面が正常粘膜で覆われ,粘膜下層に静脈・リンパ管の拡張がみられ浮腫状の結合織が増生したCMSEPと判明した.若年者の大腸憩室症に多発したCMSEPを認めた報告例は今回が初めてである." @default.
