- W2314974449 abstract "Altemeier法は百年以上にわたって直腸脱に対する標準的な経会陰手術の一つとして行われてきた．この手術は全身麻酔やサドルブロックだけでなく，局所麻酔でも行える利点がある．従来は再発率が高かったために，その適応は高齢者やリスクの高い症例に限定されていた．一方，最近では直腸肛門機能検査が普及するとともに，術前に直腸脱の病態だけでなく，随伴する排便障害との因果関係も考慮して病態診断を行い，その病態を補正する手術を選択することができるようになってきた．経肛門的直腸S状結腸切除とともに肛門挙筋形成を行うAltemeier法が標準手術として広く認知されるとともに，1990年以降の報告では術後の再発率も低くなっている．最近ではAltemeier法が低侵襲で全身状態にかかわらず選択できるだけでなく，補正すべき病態によって手術内容を臨機応変に微調整できることから，術後排便機能の補正という視点からも，その適応が見直されている．" @default.
