The purpose was to determine whether a subset of these babies given antibiotics could have their antibiotics discontinued before the traditional 48–72 hour coverage period. Their letter states “early markers of sepsis should have not only high NPV but also moderate PPV,” but we were not interested in the positive predictive value, only in the negative predictive value, which would enable us to shorten the initial antibiotic course if, as it proved, the the negative predictive value was 100%. Their study, looking at markers to try to determine which babies were infected, is only 1 of a multitude of studies trying to determine which infants have an actual infection. This was not the focus of our study. Conservatively, in the United States alone, more than one-half million infants are ruled out for sepsis at birth. Our data suggest that about one half of these infants may be able to have their antibiotics discontinued after only 2 doses than the usual 4 or 5 doses. Single white blood cells are not helpful in ruling in or ruling out sepsis as Soni et al’s data only reinforces. Our laboratory has a 2-step process for all neonatal white blood cell counts. They are initially evaluated with an automated differential, but all newborn white blood cell counts are then reviewed by a technician and a manual differential is reported. There is excellent correlation, especially for “normal” differentials. As our screens are performed at a minimum of 8–12 hours separation, they are run separately.
