Histologically, itcomprises of pure seminoma (SE) and nonseminoma (NS) at almost equal proportions. Epidemiological observations suggest a recent shift towards increasing age at diagnosis and also, a trend towards increasing relative proportion of SE. We tested these hypotheses using the data bases of two major testicular cancer units in Northern Germany. METHODS 2482 patients with histologically proven GCC diagnosed between 1976 and 2010 were retrospectively enrolled. Histology of primary tumor and age at presentation were registered in every patient. Mean age and relative proportion of SE were assessed regarding patient cohorts treated before 1990, 1990-94, 1995-99, 2000-04, and 2005-2010, respectively. Mean ages and relative proportions of SE were compared among the cohorts by employing the chi square test and ANOVA, respectively. RESULTS Both, mean age and relative proportion of SE showed a significant constant increase over time (table). The age difference between NS and SE remained between 6 to 8 years over all time periods. n Age (all patients) years Age (SE) years Age (NS) years Proportion SE vs. NS (%) < 1990 424 28 34 26 31 1990-94 292 30 34 28 36 1995-99 309 32 35 30 44 2000-04 634 34 37 31 55 2005-10 823 36 39 31 56 p-value <0.001 <0.001 <0.001 <0.001 CONCLUSIONS There is a significant and ongoing shift towards older age at first diagnosis of GCC. Accordingly, as histology and age are associated biologically, the proportion of seminoma is constantly increasing. Yet of note, also the mean age of nonseminoma patients is increasing. The reasons for this development are obscure. Based on these data, one may conclude that SE and NS must have at least partly divergent pathogenetic pathways and also, one might speculate that environmental factors are increasingly affecting the pathogenesis of GCC. © 2012 by American Urological Association Education and Research, Inc.FiguresReferencesRelatedDetails Volume 187Issue 4SApril 2012Page: e303-e304 Peer Review Report Advertisement Copyright & Permissions© 2012 by American Urological Association Education and Research, Inc.MetricsAuthor Information Christian Ruf Hamburg, Germany More articles by this author Hendrik Isbarn Hamburg, Germany More articles by this author Cord Matthies Hamburg, Germany More articles by this author Walter Wagner Hamburg, Germany More articles by this author Klaus-Peter Dieckmann Hamburg, Germany More articles by this author Expand All Advertisement Advertisement PDF downloadLoading ..." @default.
