- W2315785095 abstract "54歳女性．検診で肝機能異常を指摘され当科紹介となった．腹部超音波および腹部CT検査で胆嚢内結石と肝内胆管の拡張を認めた．Mirizzi症候群または上部胆管腫瘍が疑われ経皮経肝胆道鏡（PTCS）を施行したところ，左右肝管合流部直下に嵌頓した結石を認めた．PTCS下砕石後の胆管造影およびPTCSで，総肝管は本来の位置に存在せず，後区域胆管枝を除く肝内胆管が異常な胆管交通枝（cystohepatic duct）を介して胆嚢管に合流する胆道奇形の存在が明らかとなった．後区域胆管枝は，cystohepatic ductを介さずに単独で総胆管に合流していた．この所見より，本例は胆石がcystohepatic ductと胆嚢管の合流部に嵌頓したために，Mirizzi症候群様の所見を呈したものと考えられた．胆嚢摘出のみ施行し経過観察を行っているが，現在までに結石の再発や胆汁鬱帯所見は認めていない．" @default.
