- W2316262139 abstract "【目的】後拡張を省略した頚動脈ステント留置術（carotid artery stenting；CAS）の治療成績を検討する．【対象と方法】2010年1月より2012年4月までに当院および関連施設にて施行したCASは81患者85例であり，全例で後拡張を省略した．術後30日以内の脳卒中，心筋梗塞，死亡および術翌日のMRI拡散強調画像（DWI）における高信号の出現，術後31日以降の同側脳卒中および再狭窄について検討した．【結果】症候性病変49例（57.6％），無症候性病変36例（42.4％）であった．使用した遠位塞栓防止デバイスは，GuardWireが25例，FilterWire EZが60例である．30日以内の脳卒中は2例（2.4％）で，無症候性病変に脳梗塞を1例（2.8％），症候性病変に頭蓋内出血を1例（2.0％）に生じた．心筋梗塞および死亡例はなかった．DWI高信号の出現は12例（14.1％）であった．治療後31日以降で，同側脳卒中は1例（1.2％）であり，再狭窄は3例（3.5％），そのうち再治療を行ったのは1例（1.2％）であった．2種類の遠位塞栓防止デバイス間で，周術期脳卒中，DWI高信号の出現に有意差はなかった．【結語】後拡張を省略することはプラークの破壊を最小限にし，塞栓性合併症の低減に寄与していると考えられ，中期成績も良好であった．" @default.
