- W2316461794 abstract "【目的】TKAにおける術前・術後のVTE一次スクリーニングとしてD-dimer値測定の有用性を検討することである．【対象と方法】対象は，TKA症例のうち，術前と術後7日目にMDCTを用いてVTEを評価した，連続するOA膝128膝である．VTEの有無（VTE群とN群）とD-dimer値（術前，術後3，7日目）より，術前・術後の検査特性を評価した．なお，予防的抗凝固療法を行った29膝は術後検索から除外した．【結果】術前検索では，VTE群13膝（10％）とN群115膝との間でD-dimer値には差はみられなかった．術後検索では，VTE群は23膝（24％）となり，D-dimer値は術後3，7日目ともにVTE群が有意に高かった．術後のcut off値を10μg/mlとすると，感度は高いが特異度は低くなった．【結論】D-dimer値測定の有用性は術前検索では低く，また，術後検索では施設毎のcut off値の設定が必要であるものと考えられた．" @default.
