METHODS Relying on the American College of Surgeons National Surgical Quality Improvement Program (2005–2011) database, patients undergoing one of the 3 major urologic oncologic (prostate, bladder, renal) surgeries were identified. Self-reported smoking status at the time of surgery was used to classify patients as non-smokers, former smokers or current smokers. Procedure-specific multivariable logistic regression models assessed the association between smoking status and the risk of 30-day morbidity (complications, prolonged length of stay, re-intervention and readmission) and mortality. RESULTS Overall, 9,014 patients were identified. In adjusted analyses, smoking presaged an unfavorable prognosis with current smokers having significantly higher odds of pulmonary and renal complications following prostatectomy. Further, current smokers also had increased odds for prolonged length of stay following prostatectomy. Smokers did not seem to have increased odds for complications following cystectomy or nephrectomy; however in patients undergoing cystectomy current smokers did experience increased odds for re-intervention while former smokers had increased odds for readmission. Encouragingly though, in patients undergoing prostatectomy that had stopped smoking (i.e. had not smoked for at least a year) were not at increased odds for any of the complications when compared to non smokers and were at significantly decreased odds for complications compared to current smokers. CONCLUSIONS The effect of smoking on perioperative outcomes is procedure dependent. Current as well as former smoking negatively influences outcomes.
