Функционирование кожного барьера напрямую зависит от строения и состава системы межклеточных липидов. Липиды рогового слоя синтезируются в кератиноцитах по мере их созревания. На уровне перехода гранулярного слоя в роговой происходит выброс этих липидов (точнее их предшественников) в межклеточное пространство, где при участии энзимов начинается ферментативная сборка липидных пластов, составляющих липидный барьер. Стандартная базисная терапия не приводит к восстановлению баланса в структуре поверхностной липидной пленки кожи. Комплексная терапия одновременно с кремом Карталин способствует повышению ее эффективности, что сопровождается выраженной положительной динамикой (по индексу PASI) на фоне нормализации показателей гидролипидного барьера кожи и местного иммунного ответа. Цель исследования — изучение клинической эффективности применения крема Карталин у пациентов с распространенным псориазом с легкой и средней тяжестью заболевания. Материал и методы. Были обследованы и пролечены 100 пациентов с диагнозом Псориаз. Результаты. Крем Карталин может быть использован в виде монотерапии легкой и средней степени тяжести псориаза в амбулаторных и поликлинических условиях. Psoriasis is currently regarded as a multifactorial disease that is typically characterized by seasonal alternating periods of recurrence and remission. The skin barrier function directly depends on structure and composition of the intercellular lipid system. Horny layer lipids are synthesized in keratinocytes as they become mature. These enzymes (namely, their precursors) are released into the intracellular space at the site where the granular layer borders the horny layer; enzyme-assisted self-assembly of lipid layers composing the lipid barrier takes place. The conventional disease-modifying therapy does not recover the balance in the structure of the skin surface lipid film. The simultaneous combination therapy using Kartalin ointment makes it more effective, which is accompanied by the more pronounced positive dynamics according to the PASI score, while parameters of the hydrolipidic barrier of the skin and local immunity are normalized. Objective. The clinical effectiveness of Kartalin ointment in patients with mild to moderate widespread psoriasis. Material and methods. A total of 100 patients with psoriasis were examined and treated. The initial PASI scores were determined in all patients based on the clinico-morphological pattern. Results.
