Martin Maldonado-Duran, Washington, DC, American Psychiatric Publishing, Inc., 2002, 392 pp, $44.95. The field of infant mental health is a relatively new theoretical and empirical field that has enjoyed tremendous growth during the last decade. This field is a specialized endeavor, focusing on the psychological well-being of infants and toddlers from birth through age 3 years. One of the milestones in this field was the publication of the Diagnostic Classification of Mental Health and Developmental Disorders of Infancy and Early Childhood (Zero to Three, 1994). Many worthwhile volumes have been published since then to describe empirical research and theoretical approaches to the assessment and treatment of behavioral and emotional problems in infants and young children. These include Daniel Stern's Motherhood Constellation (1995), Handbook of Infant Mental Health (2nd edition, 2000) and many other contributions. In addition, there are national and international conferences held annually and many affiliated organizations in each state that offer educational programs focused on infant and young children's development. Dr. Maldonado-Duran's latest contribution, Infant and Toddler Mental Health: Models of Clinical Intervention with Infants and Their Families, has just arrived in this exciting field. It is a very welcomed addition, and it stands alone as a unique contribution in many respects. First, the authors offer clinical perspectives based on their own experiences, and there is richness evident throughout the volume because of the case material presented. The volume truly is a “reality-based” approach to this field. Secondly, the first section of the volume has theoretical perspectives provided from several experts who offer clear models by which to consider the clinical data presented in the second section. One theoretical chapter in the first section, by Efrain Bleiberg, provides an important discussion of the impact and perception of trauma during infancy and toddlerhood. Another chapter, by Peter Fonagy, provides a very helpful discussion of the developing sense of self during the first years of life and the role of the attachment figure in this critically important time. The second section provides several valuable presentations from expert clinicians from the United States, Japan, Mexico, and France. The first chapter, by Kathryn Barnard and her colleagues, is an important contribution that describes a successful intervention program that begins during pregnancy for at-risk mothers, particularly teenage mothers. This non-crisis-driven program appears to support the psychological growth needed for young mothers to realize positive outcomes. Although the data are incomplete, it appears to be an area for program development and longitudinal research that warrants serious attention in the future. There are many other equally rewarding chapters in this section, including a focus upon attachment disorders, by Alicia Lieberman, and a multimodal technique for infant-parent psychotherapy, by Martin Maldonado-Duran and Teresa Lartigue. Serge Lebovici and his colleagues describe their clinical intervention, “The Therapeutic Consultation.” This contribution and additional chapters provide further rich elaborations of case material that enhance the reader's understanding. The last chapter in this section provides a well-developed discussion of many young children's everyday dilemma: that of having multiple caregivers and, for some, frequent change in those caregivers. The next section of the edited volume provides three very helpful chapters on common psychophysiological disturbances involving difficulties with crying, feeding, and sleeping. Klaus Minde's chapter on sleep problems gives a clear overview of diagnostic issues and then outlines his successful treatment approach. Martin Maldonado-Duran and colleagues wrote two other chapters in this section that deal with crying and feeding. In the feeding difficulties chapter, a new approach is discussed that organizes a clinician's observations into a multiaxial scheme similar to the scheme published in the Diagnostic Classification of Mental Health and Developmental Disorders of Infancy and Early Childhood (1994). The chapter provides clear evaluation guidelines and describes many common problems with accompanying interventions. It is this pairing of problems with interventions in this chapter and throughout the volume that makes this book so worthwhile for clinicians in several different settings. The last section provides two very difficult clinical cases and the interventions used, with commentary given by the clinical experts and authors of the earlier chapters. It is a fitting close to a volume that has integrated clear diagnostic and evaluative information with case material and treatment outcome information in a smooth, concise manner. This book will serve experienced clinicians as well as the novice equally well and should be considered required reading for every medical student considering specialization in pediatrics, family practice, or child psychiatry.
