He also made the interesting comment that inflammation of organs resulted in loss of elastic recoil. Berthrong, in a singularly destructive commentary was, to say the least, sceptical of McLean's work and asserted that “small airways disease,” as we now know it, was not only not a pathogenetic mechanism in emphysema, but also that it did not exist. McLean did his research on pulmonary emphysema for a thesis for his M. D. degree in a foreign discipline (pathology; he was an internist who became a cardiologist) with minimal supervision.
