- W2316991807 abstract "わが国でも2005年10月に発症3時間以内の脳梗塞に対してrt-PAの使用がみとめられbrain attackの時代が到来した．2007年3月までに約5,700例に使用され，2,484例（平均70歳，投与前NIHSS中央値15）の解析によると，3カ月後の転帰でmRS0∼1 32%，死亡20%，症候性頭蓋内出血が5.2%であった．当院にて平成19年11月までに，63例（中央値74歳，NIHSSスコア中央値14）にrt-PA静注療法をおこなった．rt-PA投与直後のdramatic recoveryは8例（12.7%）で，7日目の転帰は，著効49.2%，改善15.9%，増悪12.7%であった．投与直後（1時間以内）の再開通率は43.5%で，再開通例では7日目の著効・改善が非再開通例とくらべ多かった（70% vs. 31%）．非早期再開通の因子として心房細動があげられた．rt-PA投与前DWI所見をDWI-ASPECTSにて評価し，1週間後の転帰についてしらべると，転帰不良例はDWI-ASPECTSが5点以下では8例中6例，DWI-ASPECTSが6点以上では41例中2例であった（p<0.0001）．以上よりrt-PA投与前のDWI-ASPECTSが5以下のばあいは，rt-PAの効果はあまり期待されないと考えられる．" @default.
