- W2317021611 abstract "Background and Purpose: In previous imaging studies the insular cortex (IC) has been identified as an essential part of the processing of a wide spectrum of perception and sensorimotor integration. However, there are no systematic lesion studies in a sufficient number of patients examining how damage to the IC affects processing of vestibular and sensory signals. We chose acute stroke patients with lesions affecting the IC in order to fill this gap. Methods: In detail, we explored signs of a vestibular tone imbalance such as the deviation of the subjective visual vertical (SVV), and we performed comprehensive sensory profiling. We applied voxel-lesion behaviour mapping analysis in 22 patients with acute unilateral cortical damage. Results: Our data demonstrate that patients with lesions of the posterior IC have deficits in SVV and temperature perception, but did not showed further vestibular or sensory deficits related to IC stroke. Beside the association at the anatomical level there is an association on the behavioural level indicating that patients with severe tilt of SVV have more severe thermoreceptive deficits. Conclusions: We conclude that the IC represents a major region responsible for vestibular and sensory processing in the brain. Thus, patients with stroke affecting the IC might have vestibular and somatosensory deficits." @default.
