- W2317263335 abstract "Background Nocardia species cause serious infections in both immunocompromised and normal hosts. When nocardiosis is diagnosed or suspected, empirical therapy is often necessary while awaiting identification and susceptibility testing. In addition, species distribution varies regionally, so an understanding of locally prevalent species may help guide therapy. Methods A retrospective review of cases of nocardiosis in a tertiary-care teaching hospital and transplant center during the past 15 years, including host characteristics, antibiotic susceptibility, and initial treatment, was conducted. Results We identified 88 cases of nocardiosis during the past 15 years. Risk factors included organ or tissue transplantation in 36 patients and other immunosuppressive conditions or chronic lung disease in 31. Most patients (73) had pulmonary infection, 8 with simultaneous involvement at another site. N. cyriacigeorgica was the most commonly identified species, followed by N. farcinica. Species distribution was similar among transplant and nontransplant patients. Most isolates were susceptible to sulfamethoxazole/trimethoprim, imipenem, and/or ceftriaxone. Conclusions At a transplant center in the American Southwest, N. cyriacigeorgica was the predominant species isolated. Initial therapy with sulfamethoxazole/trimethoprim and a β-lactam is appropriate for most patients." @default.
