- W2317434511 abstract "Decreased physical activity is a common problem in individuals coping with a recurrent pain condition. In back pain, the pain problem most common in the United States, several factors have been previously shown to contribute to the degree of pain-related limitation of physical activity, including sleep difficulties and emotional distress. However, there is notable variability in the broader literature regarding the degree to which mood and sleep affect physical activity, above and beyond the effects of pain intensity. Using daily diary data from 139 individuals with recurrent back pain, the current study examined how daily pain intensity, mood, and sleep predict daily levels of physical activity across a 28-day period. Analyses were conducted using two-level multilevel structural equation modeling, which allows for examination of relationships separately at the daily level and in differences in mean levels of these variables across participants. At the daily level, both pain intensity and sleep predicted lower levels of physical activity, explained in part through poorer mood on the same day. However, these relationships were only partially supported at the individual level; average levels of sleep quality, but not average daily pain intensity, were found to predict levels of daily physical activity, mediated by average daily mood ratings. These findings highlight the need for more reliable and time-limited measures of concomitant physical and psychological states, and highlight the relative importance of sleep and mood states in predicting daily levels of physical activity in individuals with recurrent pain. Funding provided by National Institute for Drug Abuse (K23DA031808, 3T32DA035165-02S1, K24 DA029262) and the Redlich Pain Endowment.
