- W2317943111 abstract "Rates of opioid consumption and related morbidity and mortality have escalated dramatically over the past two decades, especially in the US. Studies of health records have found evidence of increased risk for psychiatric harms, such as substance use disorders (SUDs) and depression, among recipients of prescription opioids. At the same time, however, other studies have found that opioid prescribing is more common among patients with prior SUDs, depression, and other psychiatric disorders. Thus, it is important to identify pre-existing risk for comorbid psychiatric problems and distinguish it from true adverse effects in understanding opioid harms. In the present study, we tested whether prior psychiatric factors predicted new receipt of opioid prescriptions in two national samples. We used data from the Truven Health MarketScan Commercial Claims and Encounters database, which includes 2003–2013 US private health insurance claims (N = 11,969,493 new opioid recipients aged 13–64 years; 55% female), and the Swedish population registers, which include all dispensed prescriptions from 2006–2009 for the entire resident population (N = 1,246,651 new opioid recipients aged 13–83 years; 55% female). In the US, we matched a randomly selected 25% of new opioid recipients to controls on age, sex, year of first enrollment, and number of months enrolled. In Sweden, we controlled age and sex. Individuals with prior SUD and depression diagnoses, suicide attempts, and psychiatric medication prescriptions (antidepressants, anxiolytics, antipsychotics, anti-epileptics/mood stabilizers, and drugs used in ADHD and SUDs) had modestly but consistently greater odds of receiving prescription opioids than did individuals without those diagnoses or prescriptions (odds ratios ranging from 1.10 to 1.90). In sum, in two national samples, we found support for adverse selection on the basis of prior psychiatric problems and medication treatments. Future examinations of the harms of prescription opioids should account for these factors." @default.
