Readmission due to HF is well documented at 30 and 90 days. However several patients seem to return to hospital within a very short time.The purpose of this quality initiative is to examine the cause of readmission to hospital within 14 days of discharge and to determine if changes in nursing practice could have an impact on readmission outcomes for heart failure patients.A chart review of 87 patients readmitted from April 1, 2010 to March 30th, 2011 to a large teaching hospital with the most responsible diagnosis of heart failure was conducted using the CIHI database. Of those, 27 patients were readmitted within 14 days (mean 6.4 days) Due to the following precipitating causes: 6 unavoidable non-cardiac, 6 cardiac and 15 HF related. Mean age was 74.8 years and the average day to readmission was 6.4.A brief description of the 15 HF related readmissions with demographics, clinical information and nursing care leading to the readmission will be presented as well as a review of the current HF pathway and discharge clinical tools. Challenges faced by both patient and nursing staff in regard to education will be explored, as well as potential gaps in care, implications for nursing care and solutions that could decrease readmissions in the future.
