(Bell and Saltikov, 2000). While we agree with some of her comments we feel that many of the statements do not relate to the point of the study or its conclusions – in particular: ▪Another title and publisher are given in the reference list because the article was based on Laura Mitchell's latest version of her book Simple Relaxation: The Mitchell Method for Easing Tension (1987). Vivian Grisogono rightly pointed out that a different title was used in 1977, and the misunderstanding is due to a small error on the main author's part, referencing 1977 instead of 1987. Both publications describe the same technique and therefore it is puzzling to discuss an irrelevant point. ▪Measuring the effectiveness of this technique using heart rate is highly relevant, as this is part of the relaxation response that Laura Mitchell believes can be initiated using simple relaxation. In addition the relaxation response can be initiated in everyone, particularly students in their final year of a degree course. We also acknowledged in the conclusion on page 477 that ‘future studies need to focus on measuring multiple variables'. ▪The major limitation of the study – that it included only normal subjects – was acknowledged on page 476, and it was stated that ‘the results need to be accepted with caution when making inferences about the clinical population'. Therefore we did not transfer the results of the study to patients and even reiterated this in the concluding sentence when it was stated: ‘It is important that further studies investigate the effect of this technique in … diverse patient populations.' ▪The conclusion should not adjust practice to expediency, as it was not based on a patient population. In addition no recommendation to alter clinical practice based on this study was made. ▪We do not dispute that Laura Mitchell obtained good results with her patients, and although practice should be based on treatments that benefit patients, it is fundamentally wrong to continue using a technique developed more than twenty years ago without attempting to measure its efficacy. ▪We are uncertain that reference to manual handling and Bobath practitioners is relevant in the context of evaluating a technique that requires very little hands-on treatment. In addition, breathing control is only a small part of the overall technique." @default.
