- W2320452688 abstract "The practice of neurology presents a series of ethical challenges for the clinician. These rarely have simple or straightforward solutions, but require careful consideration by the neurologist. This section of , written by colleagues with particular interest in the area of bioethics, provides a case vignette that raises one or more ethical questions related to the subject area of this issue. The discussion that follows should help the reader understand and resolve the ethical dilemma. A 53-year-old management consultant with akinetic-rigid Parkinson disease (PD) treated with high-dose dopamine agonists alone is changed to treatment with levodopa alone. He feels worse when the dopamine agonist is stopped, and it is then restarted at a moderate dose along with levodopa. At his next follow-up appointment, he says that his mood is good and, when asked about possible impulsive behavior, denies anything unusual. He says he has been going out more because work has been difficult and he needs to blow off steam. His wife, however, says he has been behaving unusually. He spends hours gambling on the Internet, where he has lost thousands of dollars. He recently used money from their children’s college education fund to purchase a vineyard, which he insists will prove profitable. He has started going to strip clubs with his male coworkers and talks about it in front of his teenaged children. The patient maintains that these activities are “normal for guys in my line of work” and that his wife is overreacting. He insists that he is in control and rejects the neurologist’s suggestion that the behaviors are caused by his PD medications and that the dose of the dopamine agonist should be reduced or eliminated." @default.
