- W2320711721 abstract "The majority of patients with the syndrome of paroxysmal cold haemoglobinuria in whom Donath and Landsteiner described their classical biphasic haemolysin test suffered from tertiary syphilis. In the last two decades acute haemolytic anaemias associated with a positive Donath Landsteiner test (DLT) have been reported also In young children following upper respiratory tract infections (URTI). Between 1975 and 1988, 7 patients (6 boys, 1 girl) between 15 and 30 months of age presented at this hospital with acute haemoglobinuria and an associated positive DLT. An URTI had been noted 3-10 days earlier. Haemoglobin nadir values were in the range 4.1 to 9.3 G/dl (Two children required transfusion). All children had negative serological tests for syphilis. Viral antibody tests failed to document a specific viral cause of the URTI. All children showed spontaneous recovery to normal haemoglobin levels. In the 4 patients with continued outpatient follow-up at this hospital the DLT became negative within 1-6 weeks of presentation. Because of the transient nature of the Donath Landsteiner haemolysin associated with viral infection a DLT is indicated early in the investigation of children with acute haemoglobinuria." @default.
