- W2320892188 abstract "2010 年に米国泌尿器科学会からVUR 診療ガイドラインが発表された。その内容は期待した実用性に乏しかったが，エビデンス不足の現状を周知させ，今後に必要な科学的診療指針の手引き書といえる。関連領域の研究が進歩し，CAKUT 理論やUrodynamics の発展，出生前診断，RI 診断の普及などは，VUR 診療の根底に大きく影響している。大きな潮流として，VUR は内科的疾患であるという認識が挙げられる。その際の予防的抗菌療法の意義に関するRCT が進行中である。幼児期以後は排尿機能発達との関連が強く，その面の治療の重要性が認識されるようになった。一部に早期から手術が不可欠な例が存在するが，特殊例である。10 年の経過で自然治癒していく例が大部分だが，もし治癒しない場合，女児ではVUR を青年期に持ち越すことは危険である。最近，内視鏡注入術というオプションが加わり低侵襲治療の道が開かれたが，将来過剰治療や不確実治療と評価されることになりかねないエビデンス不足がある。" @default.
