- W2321945979 abstract "Purpose: Optimal cardiovascular function requires appropriate coupling of the Left Ventricle (LV) and arterial system (Ventricular Arterial Coupling: VAC). The aim of this study was to investigate age- and gender-dependency of VAC assessed by 3D speckle tracking echocardiography. Methods: Transthoracic 3D full volume datasets were acquired in 335 healthy subjects without any history of cardiovascular disease (1-88 years, 169 men). Using 3D speckle tracking software (4D LV analysis, TomTec), LV volumes were determined. Estimated LV end-systolic elastance (Ees) was defined as 0.9 x systolic blood pressure (SBP)/LV end-systolic volume. Total afterload was defined by arterial elastance (Ea= 0.9 x SBP/stroke volume). Both values were indexed to body surface area (indexed Ees, indexed Ea). VAC was defined as indexed Ea/ indexed Ees. For assessing LV diastolic elastance (Ed), operant Ed was calculated (E/e' divided by stroke volume). Results: Indexed Ees was significantly increased according to the advanced aging (1st decade: 4.91±1.53 mmHg/ml/m2 to 8th decade: 7.00±2.59 mmHg/ml/m2, p<0.0001). Indexed Ea also showed a significant age-dependency (1st decade: 3.40±0.89 mmHg/ml/m2 to 8th decade: 5.38±1.07 mmHg/ml/m2, p<0.0001) resulting in the relatively narrow range of VAC values across the whole age (1st decade: 0.71±0.16 to 8th decade: 0.84±0.25, p=0.0160). Ed showed highest value in infancy (0.45±0.20/ml). The value reached to its minimal at 3rd decade (0.15±0.04/ml) and again increased up to 8th decade (0.32±0.11/ml, p<0.0001). Among 90 subjects who were older than 50 years, women had a higher indexed Ees (7.32±2.09 mmHg/ml/m2 vs. 5.86±1.60mmHg/ml/m2, p=0.0007) and indexed Ea (4.99±1.02 mmHg/ml/m2 vs. 4.63±0.93 mmHg/ml/m2, p=0.0907), resulting in lower VAC (0.73±0.21 vs. 0.83±0.18, p=0.0145) compared to men. Ed was also significantly higher in women compared to men (0.32±0.08/ml vs. 0.23±0.06/ml, p<0.0001). Conclusions: Advanced aging and female gender were associated with increase in ventricular stiffness both at systole and diastole and increase in arterial stiffness. This may play an important pathophysiological role for developing heart failure with preserved ejection fraction in elderly persons and particularly in elderly women." @default.
