- W2322122893 abstract "BPSDへの対応は，1）BPSDを適切に評価し治療対象となる症候を明確にする，2）非薬物的介入を試みる，3）十分な説明を前提とした根拠に基づく薬物療法を選択する，の手順で実施する．BPSDの評価にあたってはNeuropsychiatric Inventory Caregiver Distress Scaleなどの構造化された評価尺度を用いることが推奨される．治療法は，患者の行動を（1）認知機能障害に起因するもの（食べ物でないものを食べようとする，同じことを何度も言うなど），（2）ADL障害に起因するもの（火の不始末，尿失禁など），（3）精神症状に起因するもの（妄想に基づく他者への攻撃，易怒性など），に分類し検討する．（1），（2）に対しては環境調整，行動的介入が中心となり，（3）に対しては薬物療法も考慮する．非薬物的介入には，レクリエーション療法や音楽療法，回想法などの心理療法的アプローチから，介護者教育，環境への介入，通所サービス利用まで幅広く含まれる．薬物療法は非薬物的介入の効果が乏しい場合に試みることが原則であり，患者の苦痛や介護者の負担感，生活背景などを総合的に判断し実施する．近年，疾患を問わず認知症に共通して出現する症候としてとらえられていたBPSDを，脳損傷部位に特徴的な症候としてとらえ，その対応についても疾患別のモデルが開発されつつある．各疾患に特徴的なBPSDを知ることにより患者の行動が予測しやすくなり，先を見越した対応が可能となる．BPSDの対応において，原因疾患に目を向けること，介護者にも目を向けることとともに，患者本人に目を向けること，すなわちBPSDはその人の心の表現であり，その意味をその人の立場で理解して対応する視点を持つこと（person centered care）も常に心がけなければならない．" @default.
