- W2322232091 abstract "医療技術進歩が医療費増加の主因と言えるかを,1970～1992年の「国民医療費」と「社会医療診療行為別調査」により,四段階で検討した。(1)国民所得でデフレートした実質国民医療費は1970年代には増加したが, 1980年代には一定であった。(2)「医療技術」を投薬・注射, 画像診断・検査,処置・手術等の3種類と操作的に定義し,それらの医科医療費総額に対する割合を検討したところ,1970年代には8.3%ポイント,1980年代にも6.8%ポイント低下していた。医療技術の割合の低下は投薬・注射の低下により生じた。なお,1984年以降は,診察・在宅療養,医療技術,入院の割合も,3種類の医療技術の割合も固定化した。(3)画像診断と検査について新旧技術の変化を検討したところ,1980年代には,新技術の普及は旧来型技術を代替する形で進んでいた。(4)高度先進医療技術から保険導入された技術の医科医療費総額に対する割合はわずか0.08%にすぎなかった。以上より,わが国では,少なくとも1980年代以降は,医療技術進歩は実質医療費増加の主因ではないと結論づけられた。" @default.
