- W2322455122 abstract "要旨：【目的】嚥下造影検査(VF)の実施に合わせて，5 mlと60 mlの段階的飲水試験を施行し，誤嚥の検出や経口摂取の可否，選択された食形態との関連について明らかにする．【対象と方法】経口摂取開始時の適切な食材を選ぶことを目的にVFを施行した183名(男性107，女性76)の脳卒中患者を対象とした．平均年齢は66.9±12.1歳で，原因疾患は脳梗塞98名，脳出血49名，くも膜下出血23名，その他の脳血管疾患13名，発症からVFまでの期間は18.0±12.0日であった．方法は，まず，VF実施の直前に段階的飲水試験にて臨床評価を行い，次にゼリーや粥などの模擬食品に加え，5 mlと60 mlの液体にてVFを行った．その後，段階的飲水試験の結果と実際のVF結果との関連について検討した．【結果】臨床所見の異常にて段階的飲水試験を途中で中止したのは46名(第1段階43名，第2段階3名)であった．段階的飲水試験での異常所見はVFの液体誤嚥と有意な関連を認め，VFにて観察される誤嚥に対する段階的飲水試験の感度は85.2%，特異度は41.8%であった．飲水速度(ml/秒)や1回嚥下量(ml)と誤嚥に明らかな関連はなかった．経口摂取の可否や選択された食形態は，VFの液体誤嚥との間に関連を認めたが，段階的飲水試験の臨床評価との間には関連はなかった．【まとめ】段階的飲水試験の臨床評価は液体誤嚥の検出には有用ではあるが，その結果と誤嚥予防のための適切な食形態との間には明らかな関係を見いだせず，経口摂取の開始前には嚥下造影検査などの詳細な評価を合わせて行うべきである．" @default.
