- W2322635410 abstract "INTRODUCTION: Male factor infertility affects 40–50% of couples attempting to conceive with 15% presenting as azoospermia. The objective of this study was to measure the outcomes of microsurgical sperm retrieval as an effective treatment option for azoospermia. METHODS: A retrospective cohort of a series of microsurgical sperm retrieval cases performed at a fertility center between 2004 and 2014 was identified. Descriptive statistics summarized patient characteristics and univariate analyses examined patient factors associated with a live birth. RESULTS: Forty-two cases of microsurgical sperm retrieval were identified. No sperm was found in eight patients. Among 34 male patients who had their sperm successfully retrieved, 22 (64%) contributed to the live birth of a child. Eight couples accomplished more than one successful pregnancy. A total of 43 neonates were born as a result of 83 embryo transfers. Pregnancy was diagnosed in 51 (61%) of embryo transfers and 33 (40%) of those continued to term. Nine mulitfetal pregnancies occurred. Eighteen patients miscarried. Thirteen couples terminated their treatment after the initial failed attempt (31%). Six had no sperm retrieved and three failed to conceive after embryo transfer; three experienced spontaneous abortion, and no oocytes fertilized in one case. Statistical significance was set at P<.05. CONCLUSION: Live birth of at least one neonate was achieved in 83% of couples who conceived with the first attempt or with a consecutive attempt of in vitro fertilization or intracytoplasmic sperm injection. These data indicate that microsurgical sperm retrieval is an effective option in treatment of azoospermia. More than one reproductive attempt may be necessary to accomplish a live birth in this population." @default.
