- W2322841483 abstract "PURPOSE: A typical feature of McArdle disease (McA) is the second wind phenomenon, characterized by the sudden decrease in heart rate (HR) and the improvement of exercise tolerance after about 8 min of exercise. In the present study we investigated the effects of a first bout of exercise on the second wind during a subsequent bout. METHODS: Measurements were conducted on 5 McA patients (42 ± 19 years) during two repetitions (CLE1 and CLE2) of 6-min constant-load submaximal exercise (about 50% of the peak workload) separated by a 6-min recovery. Breath-by-breath pulmonary ventilation (V'E) and O2 uptake (V'O2), HR, cardiac output (Q'), rates of perceived exertion (RPE) were determined; vastus lateralis oxygenation indices (delta[deoxy(Hb+Mb)]), obtained by continuous-wave near-infrared spectroscopy (NIRS), were expressed as a % of maximal values obtained during transient limb ischemia, and were considered as estimates of skeletal muscle fractional O2 extraction. RESULTS: In CLE2 vs. CLE1 significantly lower values were described for V'O2 (0.92 ± 0.1 vs. 1.01 ± 0.1 L/min), V'E (28.1 ± 2.4 vs. 36.4 ± 3.3 L/min), HR (112 ± 2 vs. 146 ± 4 b/min), Q' (11.7 ± 0.6 vs. 15.0 ± 0.8 L/min) and RPE (11 ± 0 vs. 14 ± 1). Delta[deoxy(Hb+Mb)] was higher in CLE2 (6.5 ± 5 %) vs. CLE1 (-6.0 ± 4 %). In all patients a slow component of V'O2 kinetics was present in CLE1 (amplitude, as a % of the total response, 16.0 ± 3.5 %) but not in CLE2. CONCLUSIONS: In McA patients we observed a clear second wind phenomenon during the second of two consecutive 6-min constant-load submaximal exercises, separated by 6 minutes of recovery. The second wind was associated with clear signs of improved exercise tolerance. If one considers that many activities of everyday's life are characterized by bouts of exercise separated by recovery periods, the present results appear of interest also from a clinical point of view.
