Therefore having a rescue airway is essential to emergency airway management. Since these clinical presentations are rare occurrences, having simulated situations with a realistic airway model is critical. Currently, there are no commercially available simulators that adequately replicate needle cricothyrotomy and manage jet ventilation. While easy, low-cost modification to an existing simulator provided an opportunity to teach critical skills required to safely perform a needle cricothyrotomy, preliminary evaluation of validity evidence is required before next implementation. Description Presented with an immediate need during an Emergency Medicine Fellows’ course, our team modified an adult airway training piece which is used to show relevant anatomy (Laerdal® Airway Management Trainer, Laerdal Medical) to better replicate needle cricothyrotomy and manage jet ventilation. Two airway ventilaing bags were taken from the anesthesia machine and zip tied to the airway trainers. Hook and loop strips were attached to the base of the airway trainer and a fake skin was attached to cover the membrane and add fidelity. Gauze was rolled up and stuffed int he trachea to simulate an obstruction. Fifteen (15) participants were requested to complete a self-report, five-domain, 18-item instrument consisting of four-point rating scales via the web (Qualtrics). Response options had an added option of “I don’t know,” which was recoded as missing data. An additional 4-point item was used to identify participants’ global opinion of the simulator. Conclusion The highest observed averages were associated with Value as a Training Tool, Value as Testing Tool, and Relevance to Practice (all 3.7). The lowest ratings were associated with the crycothroid membrane. For example, Physical Attributes-Anatomical landmark, tactility - cricothyroid membrane and Realism of Experience-Response of cricothyroid membrane when introducing needle had observed averages of 2.7 and 2.5) respectively, indicating the crycothroid membrane had “Adequate realism, but could be improved.” The observed average of Global rating was 2.1/4.0, indicating the simulator “..requires a small tweaks before it can be considered for use in jet ventilation training.” One comment from participants was consistent with low ratings associated with cricothyroid membrane; “The membrane was tough, maybe a different material could be used.” Preliminary evidence suggests we were successfully able to modify an existing decommissioned simulator into a needle cricithyrotomy simulator using inexpensive, readily accessible materials.
