- W2323347029 abstract "Background: Lymphoid malignancies in adults with AIDS frequently have mutations and/or translocations of the c-myc oncogene. To compare the biologic features of children with AIDS and malignancies to those of adults, the Pediatric AIDS-Lymphoma Network has studied 14 cases of lymphomas and B-cell leukemias in children with AIDS for abnormalities in the immunoglobulin heavy chain gene (JH), c-myc oncogene, and presence of Epstein Barr Virus(EBV). Methods: Patients studied besides those with AIDS-associated malignancies were 12 HIV-negative children with the same lymphoid malignancies, and 5 children with AIDS and lymphoid hyperplasia. DNA from malignant tissue was extracted and amplified by the polymerase chain reaction with oligonucleotides for the JH region, the first two introns and all three c-myc exons. Quantitative PCR for EBV was performed by amplification of a portion of the BamHI W fragment. Segments of each c-myc region were analyzed by Single Strand Conformational Polymorphism (SSCP), then DNA sequencing to identify the specific nucleotide changes. C-myc-immunoglobulin gene rearrangements were established using along PCR technique. Results: A clonal proliferation of lymphocytes as noted by a single band of amplified JH was identified in 2/5 SNC, 1/4 LCL 1/3 B-ALL, and the 1 HD patient. The remainder had oligoclonal proliferations. Only 2/14 patients had mutations ofc-myc (1 each first and second exon). Ig gene-c-myc translocations were found in 1/3 B-ALL, 1/4 LCL, the MALT lymphoma and 3/5 SNC tumors. HD and B-ALL tissues were negative for EBV. Two SNC tumors had very high levels of EBV(>1×106 copies/105 cells), one of which had ac-myc mutation. The other c-myc mutation occurred in a MALT with 547 copies of EBV/105 cells. Conclusions: AIDS-associated lymphomas in HIV-infected children are rarely associated with abnormalities of the c-myc oncogene and usually do not contain EBV DNA. Further work is needed to characterize cytokine, tumor suppressor gene, or other oncogene abnormalities in the lymphoid malignancies of children with AIDS." @default.
