- W2323401913 abstract "症例は70歳，女性．主訴は右鼠径部膨隆．1カ月前より主訴を自覚し右大腿ヘルニアの疑いで当科紹介．来院時，右鼠径靱帯の足側，大腿動脈の内側に30×25mmの弾性軟な腫瘤を触知し同部位に軽度圧痛を認めた．腹部造影CT検査上，右大腿管内に虫垂の陥入が疑われ，de Garengeot herniaと診断した．全身状態より緊急性は無く，待期的に手術を施行．ヘルニア嚢を開放すると少量の漿液性腹水と共に発赤・腫脹を伴わない虫垂先端・虫垂間膜が確認された．虫垂間膜とヘルニア嚢の癒着を剥離し腹腔内に還納した．創汚染は無いと考え，PHS法を用いヘルニアを修復し手術を終了した．大腿ヘルニア内虫垂嵌頓は鼠径部ヘルニアの約0.5％とまれである．また，術前診断が非常に困難とされる．自験例では画像診断により術前診断が可能であった．また，治療は虫垂切除を施行しなかったためmeshを用いることができ，術後経過も良好であった．" @default.
