- W2323435149 abstract "【目的】大学病院における脳卒中治療および急性期リハビリ後のADL改善と転帰について，脳卒中ユニット(SU)導入前後で比較し，その意義を示すとともに，大都市医療圏における脳卒中の地域完結型の診療連携の実態と問題点について年次を追って調査した． 【方法】平成18年3月1日～平成21年3月31日に当大学病院に入院し，リハビリテーションを実施した脳卒中患者788例を年次別に，年齢，在院日数，ADL自立度(FIM)を比較した．さらに転帰先を自宅，回復期病床，療養型等病床に分類し，年次変化を調査した．【結果】在院日数はSU導入直後で短縮したが，次年度以降は再延長した．転帰先別では自宅退院群で有意に在院日数の延長が認められた．SU導入後の二次医療圏内への転院割合は回復期病床で53.7%，療養型等病床で41.8%と医療圏内への転出の困難さが目立った．【結論】大都市圏では回復期・療養型など病床数不足が顕在化し，急性期病院のSUには回復期リハビリ機能の一部が求められている．大都市圏で効率的な医療連携を行うためには，とくに療養型病床を増やす必要がある．" @default.
