- W2323580510 abstract "Increased rates of twin pregnancies during the past 2 decades, largely the result of worldwide usage of assisted reproductive techniques, have been associated with significant perinatal mortality and morbidity. The aim of this retrospective study was to determine perinatal mortality and morbidity rates in twin pregnancies according to chorionicity and gestational age at birth and to investigate the underlying factors contributing to increased rates. Medical records of 300 twin deliveries, which occurred at a single clinic between 1996 and 2005, were reviewed to obtain data on perinatal mortality and morbidity rates, zygosity and chorionicity, gestational age at delivery, method of conception, delivery route, birth weight, fetal gender, and cesarean delivery rates. Among the 16,549 deliveries reviewed, 2.9% (n = 484) were twin deliveries. Rates of perinatal mortality, adjusted perinatal mortality, fetal mortality, and neonatal mortality were 7.5%, 6.9%, 3%, and 5.8%, respectively. The overall rate of perinatal morbidity was 15.4%. The principal cause of perinatal morbidity was respiratory distress syndrome, whereas for perinatal mortality, the primary causes were prematurity, intrauterine fetal demise, and congenital abnormalities. Neonatal intensive care unit and positive pressure ventilation were required in 28% (n = 162) and 12.1% (n = 70) of newborns, respectively. Perinatal mortality and morbidity rates were closely linked with chorionicity and were found to be independent of zygosity. Perinatal morbidity and mortality were higher for women younger than 18 and older than 35 years and were not affected by intrauterine growth restriction or discordance between twins. These findings indicate that twin pregnancies should be monitored more intensively than singleton deliveries for perinatal morbidity and mortality, and similarly that monochorionic twins deserve more intensive follow-up than dichorionic twins." @default.
