Cohen and Kurzrock for their letter highlighting their description of two additional patients with a similar Grover's-like disease eruption after 2-chlorodeoxyadenosine chemotherapy for hairy cell leukemia (1). Their first patient is particularly informative, because despite the fact that he had fever, he did not have excessive perspiration, hospitalization, or occlusive immobility. Nonetheless, his eruption involved the forehead. This observation reinforces our thinking that it is perhaps some interplay of fever and antineoplastic therapy that results in this disorder. It is of interest that in a recent preliminary report by Antley et al., the lesions of classical Grover's disease were assessed by immunohistochemical techniques for carcinoembryonic antigen (CEA) and cytokeratin 7 to evaluate the relationship of the acantholysis to acrosyringia (2). In four of five patients, the acantholysis was not associated with the eccrine outflow tract, and in the fifth patient, only inconsistent association was seen. Because of this, similar studies were done in three of our patients' biopsies. In contrast to Antley et al., our cases showed the acantholytic vesicles adjacent to or above acrosyringia, but the acrosyringial keratinocytes themselves were relatively uninvolved. In some fields, an intact acrosyringium transversed the vesicle (Fig. 1). This finding implies a closer association with eccrine secretions and the acantholysis in the chemotherapeutic setting. The ultimate pathogenesis of these lesions and their relationship to more classical Grover's disease is yet to be defined.
