• W2325020980 abstract "Cytomegalovirus (CMV) infection is a risk-factor for acute and chronic rejection but does not account for rejection in most lung transplant recipients. The majority of adults are latently infected with multiple viruses, including herpes simplex virus-1 (HSV-1), Epstein-Barr virus (EBV), human herpes virus-6 (HHV-6) and CMV. Upon immunosuppression, these viruses may become reactivated and lead to active disease. We hypothesized that transplantation with lungs that were latently infected with one or more of these viruses might be associated with decreased recipient survival. Genomic DNA from human lung tissue retrieved from donors prior to harvest was isolated and quantitative PCR was performed for HSV-1, HSV-2, EBV, HHV-6 and CMV. Only lung tissue from the donors of deceased recipients was included, the primary endpoint being duration of recipient survival. Seventy-five donor-recipient pairs were included in this cohort, which were part of a larger cohort of lung tissues obtained as part of a study of rejection. Our results showed a single donor was positive for CMV and 13 donors were positive for HHV-6. The recipient of the CMV-positive lung did not have any episodes of CMV viremia or CMV positive shell vial cultures but expired 40 days after transplant from a bowel perforation. The recipients of HHV-6 positive lungs were compared to 62 recipients whose donors did not have HHV-6. The positive recipients were found to have a shorter median survival; with HHV-6 positive recipients surviving for a median of 113 days (range 6-3115 days) versus HHV-6 negative recipients surviving for a median of 579 days (range 4-2807 days). This difference was statistically significant with a p=0.002 by the Wilcoxon signed rank test. These data support that the presence of donor-derived viral DNA shortens recipient survival. Further studies are needed to define these findings, including studies of additional viruses and the examination of donor lungs of surviving recipients." @default.
