• W2325344299 abstract "American Indians are a vulnerable yet understudied ethnic minority population. American Indians have a high prevalence of metabolic and vascular conditions, including diabetes and stroke, both of which are well-established risk factors for dementia. Yet, almost nothing is known about incidence or risk factors for dementia in this group. We examined the epidemiology of dementia and role of vascular risk factors over 14 years among American Indian members of a healthcare delivery system with equal access to care. 4,543 American Indian members of Kaiser Permanente Northern California (KPNC) aged ≥64 years with no prevalent dementia diagnoses as of January 1, 2000 were studied. Dementia diagnoses were collected from medical records January 1, 2000-December 31, 2013, with a mean follow up time of 9.1 years. Baseline vascular risk factors including diabetes, depression, hypertension, stroke, and cardiovascular disease were abstracted from medical records January 1, 1996-December 31, 1999. We calculated age-adjusted dementia incidence rates and evaluated associations of vascular conditions and dementia risk with sex-adjusted Cox proportional hazards models with age as the timescale. At baseline the mean age was 73.5 years (range 64-97) and 54.3% female. During the 14 year follow-up, 26.9% of the cohort (n=1,224) were diagnosed with dementia. Nearly 29% (n=1,311) died without a diagnosis of dementia. The age-adjusted dementia incidence rate was 20.9/1,000 person-years (female: 21.1/1,000 person-years; male: 20.5/1,000 person-years). There was a high rate of vascular conditions: 29.6% diabetes, 14.0% depression, 54.4% hypertension, 9.5% stroke, and 28.2% cardiovascular disease (Table 1). Those with depression were 42% more likely to develop dementia, those with stroke were 32% more likely to develop dementia, and those with diabetes were 24% more likely to develop dementia (Table 1). This is the first epidemiological study of dementia incidence in American Indians. Over 14 years, cumulative incidence of dementia was 26.9%, which is 5% higher than among white KPNC members. Though the prevalence of vascular conditions was high, the magnitude of the associations on dementia tended to be weaker than in other populations. Future studies are needed to understand risk and protective factors of dementia in this high-risk minority population." @default.
