If malleolar position results in talar shift, tibiotalar contact pressures increase considerably. An increase in asymmetric contact forces can lead to early ankle degenerative disease and arthrosis. Initial management involves a full clinical and radiographic evaluation. An understanding of the radiographic osseous relationships of the ankle is necessary for correct diagnosis and treatment planning. Consideration should be given to the degree of ankle arthrosis already present as well as each individual’s comorbidities and functional demands. The most common deformity seen after failed treatment for ankle fracture that leads to talar tilt is lateral malleolar shortening and external rotation. Revision fibular fixation through the original fracture line or a remote osteotomy can restore anatomic length and rotation to the lateral malleolus. Failed management of a distal tibiofibular syndesmotic injury also can cause nonanatomic talar position and require operative reduction and stable fixation. Less frequently, talar malposition is the result of failed union of the medial or posterior malleolus. In these ankles, revision fixation or correction through a planned osteotomy should be performed. The goal of any treatment plan for the failed ankle fracture is a stable concentric ankle joint. Salvage procedures, such as arthrodesis, rarely are indicated unless end-stage arthrosis is already present." @default.
