- W2327536478 abstract "Endometrial adenocarcinoma is the most common malignancy of the female reproductive tract. The American Cancer Society estimates that for 1997 there will be 34,900 newly diagnosed cases of endometrial adenocarcinoma and 6,000 deaths from this disease.1 Epidemiologic studies have identified some of the risk factors for endometrial cancer such as postmenopausal status, obesity, and unopposed exogenous estrogen use. Screening for endometrial cancer has not been feasible because only one half of patients have identifiable risk factors. The other half of patients appear phenotypically to be at low risk: thin or of average weight, no history of exogenous hormone replacement, 20-25% are perimenopausal, and approximately 5% are premenopausal. It has been suggested by several authors that there may be two distinct entities of endometrial cancer.2-4 One form is estrogen related, tends to be more well-differentiated, and, in general, carries a better prognosis. The second type is not related to estrogen use, is more poorly differentiated, and behaves more aggressively.3 With recent advances in molecular genetics, the etiology of endometrial cancer now is being investigated to determine if there is a genetic predisposition to either or both of the different risk categories. Patients may be at risk for developing endometrial cancer because of excess exogenous or endogenous estrogen stimulation or by virtue of carrying a germline mutation responsible for causing a hereditary cancer syndrome." @default.
