- W2328266410 abstract "びまん性汎細気管支炎 (DPB) は, あらゆる抗生物質および補助的治療にもかかわらず, 致死的な慢性気道炎症疾患であったが, エリスロマイシン少量療法がその予後を劇的に改善して以来, 標準的な治療法となった。我々は初診年度別に498名のDPB患者を3つのグループに分け (a: 1970～1979年, b: 1980～1984年, c: 1985～1990年) 比較した。患者は慢性の咳嗽, 喀痰, 呼吸困難, 両側中下肺野の辺縁不正な小粒状影, DLcoの低下を伴わない閉塞性換気障害, 寒冷凝集素上昇, リンパ球集簇, 泡沫細胞の集簇を伴った細気管支の閉塞を特徴とする診断基準に基づいて, 診断された。c群の生存曲線はb群 (P<0.0001), a群 (P<0.0001) にくらべ有意に生存率が改善した。近年の話題は, もっぱらエリスロマイシン療法の作用機序に関心がもたれている。気道の水分泌, 粘液分泌の抑制, IL-8の抑制を伴った好中球の炎症部位への集積の抑制, さらにその結果好中球エラスターゼ, 活性酸素種の抑制などが報告されている。気道炎症と慢性気道感染症との関係を考察していくことが, 今後の課題である。エリスロマイシン少量療法は抗菌活性が期待されるのではなく, 抗炎症作用によるものであり, さらにその機序が解明される必要がある。" @default.
