- W2328685766 abstract "Burch retropubic urethropexy (RPU) is increasingly performed as a laparoscopic procedure because it provides a faster and less painful postoperative recovery and shorter hospital stay than the abdominal approach with success rates equivalent to abdominal procedures. It is not established that these advantages exist when additional surgical procedures are performed concurrently with RPU. This retrospective study was conducted to compare the morbidity and cost of laparoscopic (N = 76) and abdominal (N = 143) Burch retropubic urethropexy performed concurrently with vaginal prolapse repair or hysterectomy. In the laparoscopic procedures a transvaginal route was usually used for pelvic reconstructive procedures. A transvaginal or abdominal route was used for pelvic repair when open RPU was performed. In general, patients who underwent laparoscopic procedures were younger with less pronounced prolapse than those who had open procedures. They were also less likely to have undergone previous hysterectomy or retropubic anti-incontinence procedures. Vaginal hysterectomy, salpingo-oophorectomy, and McCall culdoplasty were more likely to be performed with the laparoscopic RPU than with the open approach (61% vs. 30%, P = .001; 62% vs. 27%, P = .001; and 95% vs. 82%, P = .008; respectively). Conversely, those who underwent an open RPU had a higher percentage of anterior (66% vs. 49%) and posterior repair (96% vs. 67%) than the women in the laparoscopic group (P = .011 and .001, respectively). In five women, the procedure was converted from laparoscopic to abdominal surgery; these procedures are included in the laparoscopic group for purposes of morbidity and cost comparison. Other than slightly more symptomatic hematomas reported in the laparoscopic group, there were no differences in the complication rate associated with either procedure. Similarly, the groups had no significant differences in blood loss, operative time, hemoglobin change, length of hospital stay, or hospital charges (Table 1)." @default.
