This study has significant strengths including the use of molecular methods to enhance the detection of both, pneumococcus, using a polymerase chain reaction assay in the blood and pleural fluid, and respiratory viruses by using comprehensive polymerase chain reaction assays. Their work illustrates the enhanced diagnostic capability derived from the application of these molecular assays and provides a strong argument in favor of their routine application in clinical care.2,3 Their results confirm our observations that the detection of a viral coinfection does not appear to be associated with worse clinical outcomes.4 However, the number of patients included in their study, as well as in our cohort, is relatively small to be able to generalize this observation. They found that rhinoviruses were the most frequent respiratory viral pathogens detected in this patient population. This finding confirms a number of studies that have identified rhinoviruses as the most common respiratory virus detected in patients with community-acquired pneumonia.5–7 Although the role of rhinoviruses in lower respiratory tract infections is clearly established, there is still much to learn about their pathogenic capabilities and their role as copathogens. Understanding how, when, and in which type of hosts do the different respiratory viruses cause upper respiratory infection, wheezing, or pneumonia, and whether they enhance the severity of the disease in children with bacterial pneumonia are the critical questions that deserve major research efforts.
