Previous studies have reported that 2% to 3% of pregnant women have subclinical hypothyroidism. The benefits of universal antenatal screening are unclear, and it is controversial whether routine thyroid screening of asymptomatic pregnant women is warranted. There are only limited data on current national thyroid testing rates and positivity during pregnancy. This study was designed to estimate the current thyroid testing rate and positivity during pregnancy and postpartum in a large, national sample of pregnant women in the United States. Conducted during 36 months from 2005 to 2008, the study included 502,036 pregnant women aged 18 to 40 years. Among pregnant women who tested positive for hypothyroidism, testing and positivity rates of thyroperoxidase antibody (TPO Ab) and results of free T4 tests were examined. The control group was composed of pregnant euthyroid women (without elevated thyroid-stimulating hormone levels). The primary study outcomes were testing rates and the positivity rate (prevalence) for hypothyroidism during pregnancy and postpartum. These rates were measured using assay-specific, trimester-specific reference intervals. Rates of TPO Ab and free T4 testing also were calculated. Among the 502,036 women in the sample, 117,892 (23%) were tested for gestational hypothyroidism (defined as both subclinical and overt hypothyroidism). Of those tested, 15.5% (18,291/117,892) tested positive for gestational hypothyroidism. Testing for gestational hypothyroxinemia was performed in 24% (22,650/93,312) of euthyroid women and 33% (6,072/18,291) of hypothyroid women. There was a positive test for gestational hypothyroxinemia in 0.2% (45/22,650) among the euthyroid women who were tested and 2.4% (144/6072) among the tested hypothyroid women. A TPO Ab test was performed in 0.3% (276/93,312) of the pregnant euthyroid women and 15% (41/276) of these tested positive; 0.66% (120/18,291) of the pregnant hypothyroid women received a TPO Ab test and 65% (78/120) of these tested positive. Within 6 months postpartum, only 20.7% (1873/9063) of the hypothyroid women had returned for thyroid screening; 11.5% (215/1873) of these were diagnosed with postpartum hypothyroidism. These findings show a 15% prevalence of subclinical hypothyroidism in pregnant women that is substantially higher than the commonly cited prevalence of 2% to 3%." @default.
